アメリカのUrgent Careユニットに行く機会がありました。いわゆる救急ではなく、”時間外診療”のようなところ。
先週の土曜、横断歩道を渡っていた時に道路の凸凹に足をとられ、ホームベースを目指す勢いで見事に上半身から着地。左手を道路につき、額を擦りむいてしまいました。
月曜日まで待って病院に行こうと思っていたのですが、左掌がどんどん腫れてハンバーグのようになってきたため、保険会社に電話し、保険情報を事前にFaxで病院に送信してもらってから、日曜日でも開いていたUrgent Care Unitに行ってみました。
4時間ほど滞在する間、待合室で他の人達の様子を観察したところ、ほとんどの人が怪我で来院。中には保険にかかっていない人もいて、”病院のポリシーにより、前金で＄200ドル払っていただかないと診察出来ません”と、ハッキリ言われていました。cash up frontという言葉は、映画やドラマでドラッグの売人がよく使う言葉として覚えたフレーズ。 この言葉を病院の受付で聞くとは思っておらず、かなり驚きました。
私の保険は治療費1000万以上になってもカバーされるはずなのですが、アメリカでは、保険会社の審査でハネられた部分は自費になるとのこと。今回はレントゲン写真を4枚撮り、額の擦り傷が汚いということで、破傷風の予防接種まで。そして左親指と手首を固定するスプリンターをしてもらいました。確か、レントゲンを撮って読影されると10万円くらいするはず…。破傷風の予防接種は治療じゃないけどカバーされるのか！？日本と違い、診察や検査の単価が高いので、検査をされるたびにドキドキでした。
治療費は、日を改めて請求が来るとのこと。全額カバーされるとは思うのですが、一体いくらになるのか恐ろしいです。知人の話では、赤ちゃんを落っことしてしまい、念のため頭のCTを撮影してもらったら、100万円請求が来たとのこと。治療はなく、検査のみで・・。幸い赤ちゃんに怪我はなく、保険も全額おりたそうですが、改めてアメリカでの保険の大切さを実感したのでした。
コチラのページによると、州の法律により、Emergencyの場合は支払い能力にかかわらず治療が受けられるとのこと。私が行ったのは、walk-inのUrgent Careだったので前金が必要だったのかもしれません。他の州のUrgent Careのページも調べてみたところ、”Under prompt pay discount program a charge of $98.00 is collected at the time of check-in.” と書いてありました。金額は違いますが、check-in の時に支払うとのことなので、やはり前金のようです。
具合が悪い時に、とりあえずお金の心配をせずに受診出来る日本の制度はありがたいと思いました。
A hospital in the U.S. is the last place a foreigner would want to visit. The system is ambiguous to those who rarely visit, and the cost it might incur is unreasonably high. I wanted to avoid visiting that place if I could, but my palm started to swell after I fell to the ground in a pedestrian crossing last week.
I called my health insurance company, and the operator recommended an urgent care hospital in my town. She was kind enough to send my insurance information to the hospital via FAX, and she assured me that there would be no payment required at the hospital.
During the 4-hour stay at the urgent care unit, I observed people in the reception area. Most of the people came to the hospital for unexpected injuries, and some of them didn’t have health insurance. According to the hospital policy, they were required to pay $200 before being treated; otherwise, the doctor wouldn’t see them.
I was surprised to hear the words “cash, up front” in the conversation, because I had memorized these words spoken by drug dealers in dramas and movies. In Japan, where everybody is supposedly covered by health insurance, even doctors in private offices can’t refuse the patients, even if they don’t have any money. I didn’t expect doctors and gangs to employ the same kind of collecting system in America.
